
The Collabria Day Program
Our program touches on each of the seven dimensions of wellness: emotional, environmental, intellectual, physical, social, spiritual, and vocational.
Underlying each care plan and all of our programming is our deep aspiration to create an environment in which each participant feels deeply respected, valued, and empowered.
Early stage memory loss

Licensed Collabria Day Program
Provides state-of-the-art nursing care, rehabilitative therapies, social service support, social engagement and stimulation for adults suffering from chronic illness and/or dementia.
Individualized Plan
Each plan is geared to maximize independence, improve management of chronic conditions, enhance quality of life, and prevent hospitalization and/or premature nursing home placement.
Caregiver Respite & Counseling
Provides transportation to and from facility, giving you a needed break from caregiving responsibilities. In addition we offer free counseling to help with the stress that often accompanies caregiving.

Falls are the leading cause of fatal and non-fatal injuries for older Americans. But due to our physical assessment, state-of-the-art equipment, exercise maintenance and expert physical and occupational therapy programs, our Collabria Day Program participants have a reduced risk of falling.
In 2013, 2014 and 2015, a gait and balance assessment tool showed 95%-100% of Day Program participants scored “stable” or “improved” after participating in our therapy programs.
